Evaluations

Evaluations are used to clarify behaviors, diagnoses, and assist in treatment planning. An evaluation includes in-depth clinical interviews, multiple assessments, and potentially talking with friends/family/teachers that are close to the person being evaluated. The clinical interviews and assessments given are dependent on the unwanted and unhealthy behaviors present and the potential diagnoses. Evaluations can be compared to X-rays in the medical field; they help doctors understand if there is an issue and where.

Disordered Eating Evaluation ($750)

  • One (1) 75 minute interview session

  • Assessments:

  • 30 minute consultation with current treatment provider (optional)

  • Expansive summary of diagnostic impression, assessment results, treatment recommendations, and resources (printed and electronic)

  • 50 minute follow-up session

Personality Disorder Evaluation ($1,250)

Substance Use Disorder Evaluation ($750)

Spiritual Abuse Evaluation ($750)

  • Two (2) 50 minute interview sessions

  • Assessments:

    • Spiritual Abuse Scale (SAQ)

    • The Religious and Spiritual Struggles (RSS)

    • Institutional Betrayal Questionnaire Version 2 (IBQ.2)

  • 30 minute consultation with current treatment provider (optional)

  • Expansive summary of diagnostic impression, assessment results, treatment recommendations, and resources (printed and electronic)

  • 50 minute follow-up session

Suicidal Ideation Evaluation ($500)

  • One (1) 75 minute interview sessions

  • Assessments:

    • BSS: Beck Scale for Suicide Ideation

    • The Columbia-Suicide Severity Rating Scale (C-SSRS)

    • Patient Health Questionnaire-9 (PHQ9)

    • Suicide Assessment Five-Step Evaluation and Triage (SAFE-T)

  • 30 minute consultation with current treatment provider (optional)

  • Summary of assessment results, treatment recommendations, suicidal risk level, and resources (printed and electronic)

  • 50 minute follow-up session
